Jess G Yoga, LLC Event Contract
AGREEMENT OF WAIVER OF LIABILITY, INDEMNIFICATION, AND MEDICAL RELEASE
By signing below and participating in Jess G Yoga, LLC’s yoga event, you represent and warrant that you are in good physical health and do not suffer from any medical condition which would limit your participation in the yoga event. You further understand that it is your responsibility to consult with a physician prior to and regarding your participation in this yoga event. You also understand the risks associated with the activities offered by Jess G Yoga, LLC’s and agree that you will follow all instructions so that you may safely participate in the event and/or other activities.
You hereby WAIVE AND RELEASE Jess G Yoga, LLC’s, its owners, officers, employees, and instructors from any claim, demand, or cause of action of any kind resulting from or related to your participation in the yoga event or other activities offered by Jess G Yoga, LLC’s. You understand and acknowledge that you are fully responsible for any and all risks, injuries, or damages, known or unknown, that might occur as a result of your participation in this yoga event. By signing below and participating in the yoga event you agree that you have read the above release and waiver of liability, fully understand its content and that you are legally competent to sign and voluntarily agree to the terms and conditions stated above.
Terms & Conditions:
This Contract is in force, unless canceled by you within 24 hours of this signed agreement. Please read and acknowledge below.
(a) You agree to pay for the Jess G. Yoga, LLC yoga event in full at the time of purchase.
(b) You hereby certifies that they are the holder of the credit card, or an authorized signer on the bank checking account provided.
(c) You agree to the following cancellation & refund policy:
- If you cancel within 24 hours of the time of purchase, you will receive a full refund;
- If you cancel after 24 hours of purchase and before 14 days of the scheduled event, you will receive a full refund minus a $50 administrative fee;
- If you cancel within 14 days of the scheduled event, no refund will be provided.
(d) You understand that there may be additional charges if I choose to participate in additional amenities provided at the yoga event.
PHOTO & VIDEO RELEASE. I acknowledge and agree that photographs and/or videos may be taken of me while you are involved in the Jess G. Yoga, LLC yoga event. I hereby grant Jess G. Yoga, LLC, its legal representatives, successors, and assigns, the irrevocable and unrestricted right (a) to use my likeness in any manner and medium and to publish photographs and/or video images of me for editorial, trade, advertising, commercial, and any other business purpose including, but not limited to, as evidence in litigation, and (b) to edit, alter, copy, copyright, exhibit, or distribute the same without restriction. I hereby waive the right to inspect or approve the finished product containing my likeness and waive any right to payment or other compensation arising from the use of my likeness. I hereby release the Released Parties from all claims and liability relating to any photos or videos.
Liability for Personal Property:
Jess G. Yoga, LLC shall not be liable to you for any personal property that is damaged, lost or stolen while on or around the Event Facility including, but not limited to, a vehicle or its contents or any property left in a locker. You shall be liable to Jess G. Yoga, LLC for any damage to the Event Facility and any equipment, furniture or fixture located thereon caused by you.
By signing below and participating in Jess G. Yoga, LLC’s yoga event, you agree that you have read the above release and waiver of liability, fully understand its content and that you are legally competent to sign and voluntarily agree to the terms and conditions stated above.
